Position Overview:
This role is eligible for the Education Debt Reduction Program (EDRP), a reimbursement initiative for student loan payments. Applicants must fulfill specific eligibility criteria as per VHA policy and submit their EDRP application within a designated timeframe after appointment.

Eligibility:
This position is open to:
  • Current federal employees in excepted service.
  • Internal candidates from the agency.

Key Responsibilities:
The Pharmacist (Hematology/Oncology) will undertake a variety of duties, including but not limited to:
  • Delivering advanced clinical services within a large healthcare facility that has active teaching affiliations, necessitating a high level of clinical pharmacy expertise.
  • Formulating therapeutic care plans tailored to individual patient needs, which involves establishing treatment goals, selecting suitable medications, and determining appropriate dosages.
  • Monitoring and evaluating the effects of drug therapies, including potential adverse reactions and interactions, by analyzing physical and laboratory data.
  • Providing recommendations to enhance medication therapy, mitigate adverse drug events, and minimize medication errors.
  • Engaging with patients through various communication methods to gather medical histories and assess for potential drug interactions and compliance issues.
  • Documenting patient medication histories and advising on the correct use of medications to ensure adherence to therapy.
  • Acting as a resource for drug information, offering advanced knowledge and expertise to healthcare professionals regarding all aspects of patient medication management.
  • Participating in the education and training of pharmacy staff, students, and other healthcare personnel as necessary.
  • Maintaining current knowledge of hematology/oncology therapeutics and innovations in pharmacy practice.
  • Contributing to the development of clinical guidelines and quality assurance initiatives to improve the safety and effectiveness of medication therapy.

Work Schedule:
Monday to Friday, with variable shifts and rotating holidays.

Travel Requirements:
Occasional travel may be necessary to fulfill job responsibilities.
