Hospice Care Specialist

We are seeking a skilled Hospice Nurse Practitioner to join our team at Heartland Hospice, a member of the Gentiva family. Our company is dedicated to providing physical, spiritual, and emotional support to patients and their families.
As a Hospice Nurse Practitioner, you will be responsible for gathering clinical information to support the hospice diagnosis and prognosis. You will communicate your findings to the hospice physician responsible for certifying or recertifying the patient's hospice eligibility.
The scope of practice for the hospice NP is limited to those tasks necessary to complete hospice F2F encounter visits. You will perform and complete physical assessments for the F2F encounter within the regulatory time frames established by CMS.
You will provide clinical findings, including review of the medical treatment plans, diagnostic tests, lab results, and medication lists to the physician who is recertifying the patient for hospice eligibility. You will make appropriate recommendations to the IDG based on findings of the F2F assessment.

Requirements:
- Master's or higher degree in Nursing
- Completion of a graduate-level nurse practitioner education program accredited by a national accrediting body
- Current state license as a registered nurse
- Current state license or certificate as an advanced practice nurse
- Current national certification as a Nurse Practitioner (ANCC or AANP)
- Must have a collaborative agreement in place
