At SSM Health, we're seeking a dedicated Nursing Care Specialist to join our team of healthcare professionals. As a key member of our hospice care team, you'll play a vital role in providing direct nursing care to patients in need.

Key Responsibilities:

  • Develop and implement personalized patient care plans, ensuring seamless coordination with our multidisciplinary team.
  • Monitor patient progress, recording and communicating vital information to healthcare professionals as needed.
  • Collaborate with patients, families, and healthcare teams to ensure comprehensive care and support.
  • Assess and coordinate patient discharge planning needs, ensuring a smooth transition to the next stage of care.
  • Work in a fast-paced environment, maintaining a high level of alertness and safety at all times.

Requirements:

  • Graduate of an accredited school of nursing
  • No prior experience required, but a passion for delivering exceptional patient care is essential
