Hospice Care Nurse Practitioner Role

We are seeking a compassionate and skilled Certified Registered Nurse Practitioner to join our team at Affinity Hospice. As a CRNP, you will play a vital role in providing exceptional care to our patients and their families.
Key Responsibilities- Work collaboratively with the Hospice Care Medical Director and other team members to deliver high-quality hospice care.
- Establish individualized plans of care for each patient, taking into account their unique needs and goals.
- Provide medical care, palliation, and management of terminal illnesses in compliance with state Nurse Practice Act and under the supervision of a collaborating physician.
- Complete history and physical examinations, ordering and interpreting laboratory and diagnostic tests, diagnosing health conditions, and documenting findings in patient medical records.
- Manage health conditions and terminal illnesses by developing individualized plans of care, prescribing medications and/or treatments, obtaining consultations or making referrals, and coordinating hospice care services.
- Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
- Current nursing licensure in the state and CPR certification.
- Masters Degree with a minimum of 1 year experience as a Nurse Practitioner and 1 year hospice experience.
- Accreditation from an approved certifying body for advanced practice nursing as required by the state.
- Reliable transportation to travel to patient homes as needed.
