Hospice Care Specialist

We are dedicated to providing compassionate and high-quality care to patients and their families in the comfort of their own homes. As a Hospice Care Specialist, you will play a vital role in delivering exceptional patient care and support to those in need.

Responsibilities:

  • Conduct Face-to-Face encounters with patients to gather clinical information and support hospice diagnosis and prognosis.
  • Communicate findings to the hospice physician responsible for certifying or recertifying patient eligibility.
  • Perform physical assessments and provide clinical findings to the interdisciplinary team.
  • Make recommendations to the IDG based on findings of the F2F assessment.
  • Notify the hospice agency when unable to complete a F2F visit according to schedule.

Requirements:

  • Master's or higher degree in Nursing.
  • Completion of a graduate-level nurse practitioner education program accredited by a national accrediting body.
  • Current state license as a registered nurse.
  • Current state license or certificate as an advanced practice nurse.
  • Current national certification as a Nurse Practitioner (ANCC or AANP).
  • Collaborative agreement in place.
