Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team. As a Registered Nurse, you will be responsible for providing high-quality care to patients and their families, and working collaboratively with our interdisciplinary team to develop and implement individualized care plans.

Key Responsibilities
  • Provide initial and ongoing assessment of the impact of the terminal diagnosis on daily living and the patient's physical, functional, spiritual, psycho-social, and environmental needs
  • Partner with an interdisciplinary team to implement and manage plans of care, determining scope, priority, and frequency of services based on patient/family needs, and recommending revisions as necessary
  • Consult with and educate the patient/family regarding the disease process, self-care techniques, end-of-life care, and ethical concerns
  • Communicate with the medical director and other clinicians regarding changes in patient needs and outcomes of intervention
  • Prepare and submit documentation of observations, findings, and services provided

Qualifications
  • Must have an active RN license
  • Have a valid driver's license, auto insurance, and reliable transportation for the purpose of traveling to our patients
  • Strong verbal and written communication skills
  • Ability to complete patient charting on a daily basis
  • A passion for providing care and a desire to take the next big step in your career
