Caregiver - Community Health Specialist

About the Role:
We are seeking a compassionate and dedicated Community Care Worker to join our team at Brighton Hospice. As a Community Care Worker, you will play a vital role in providing high-quality patient care in a variety of settings, including homes, facilities, and hospitals.
Key Responsibilities:
- Assist patients with daily living activities, such as personal hygiene, bathroom needs, and physical activity
- Support patient teaching and other procedures based on patient plans of care, supported by RNs, Therapists, Social Workers, and Chaplains
- Administer emergency first aid when needed
