Hospice Nurse

Job Summary:
As a Hospice Nurse with SSM Health, you will play a vital role in providing compassionate care to patients and their families. Your primary responsibility will be to assess and coordinate patient care needs, working closely with the healthcare team to develop and implement effective care plans.
Key Responsibilities:
- Assess patient needs and develop individualized care plans
- Coordinate with healthcare team members to ensure seamless care delivery
- Provide skilled nursing care, including wound care, IV management, and medication administration
- Document patient progress and communicate with healthcare team members
- Collaborate with patients and families to address their unique needs and concerns
Requirements:
- Graduate of accredited school of nursing or equivalent
- Valid RN license in applicable state
- Basic Life Support (BLS) certification
- Ability to work in a fast-paced environment and prioritize multiple tasks
Work Environment:
As a Hospice Nurse with SSM Health, you will work in a variety of settings, including patient homes, hospitals, and long-term care facilities. You will be required to travel to patient locations, which may involve driving and working in different environments.
