The Younker 8 unit is a 23-bed Medical Surgical facility that offers a comprehensive learning environment while providing care for patients with diverse diagnoses, particularly those with pulmonary issues. This unit fosters the development of critical thinking and nursing independence within a collaborative team. Our nursing staff is empowered to advocate for advancements in the nursing field. Through partnerships with resident physicians, governance councils, and nurse residency initiatives, we apply evidence-based practices to enhance patient care.

As an LPN within this framework, you will deliver care to a designated group of patients, employing the full range of skills acquired through your training. With the guidance of an RN and other team members, you will perform tasks within your professional scope, including but not limited to:

  • Delivering safe, patient-centered care under the supervision of an RN.
  • Collaborating with a multidisciplinary team.
  • Conducting ongoing assessments and monitoring vital signs.
  • Administering medications as prescribed.
  • Providing evidence-based patient education from the point of admission.
  • Documenting daily progress towards care objectives.
  • Performing bedside procedures (e.g., foley catheter insertion, NG tube placement, dressing changes, drain management).
  • Facilitating discharge education and reviewing instructions with patients and caregivers.
  • Maintaining accurate documentation in the electronic medical record.

Key Responsibilities

Clinical Excellence & Patient Care

  • Demonstrates the knowledge and clinical skills necessary to care for individuals with physical disabilities and chronic illnesses.
  • Engages in patient and family education activities, utilizing appropriate resources to implement personalized teaching and discharge plans.
  • Provides hands-on nursing care by applying the nursing process to achieve optimal patient outcomes.
  • Supports and reinforces the care provided by other team members.
  • Offers direction and supervision to unlicensed assistive personnel, demonstrating clinical judgment appropriate to the LPN scope of practice.
  • Coordinates nursing care activities in collaboration with the interdisciplinary rehabilitation team to meet overall goals.
  • Exhibits effective communication skills to build rapport with patients, families, and healthcare team members, ensuring compliance with legal documentation and reimbursement requirements.
  • Maintains a strong commitment to the multidisciplinary healthcare team.
  • Contributes to the assessment of patients' physical, psychological, sociocultural, and spiritual needs, as well as their educational and discharge requirements.
  • Implements the interdisciplinary care plan within the LPN scope of practice, providing nursing care and education as needed.
  • Participates in the evaluation of nursing care and modifies plans as necessary to achieve measurable goals.
  • Engages in interdisciplinary team processes during conferences and meetings, contributing to team decision-making.
  • Collaborates with healthcare professionals to ensure optimal recovery opportunities for patients.
  • Communicates effectively to facilitate teamwork and group processes.
  • Advocates for patients' and families' rights in care decisions.
  • Practices in accordance with federal and state regulatory standards.

Clinical Excellence & Patient Education

  • Reinforces patient education regarding disease processes and teaches self-care techniques to promote wellness.
  • Assists patients and families in developing self-management skills and decision-making capabilities.
  • Supports the educational efforts of the nursing staff and other healthcare professionals, providing resources for evolving patient needs.
  • Prioritizes patient and workplace safety by preventing errors and reporting concerns promptly.
Qualifications

Minimum Requirements

Essential qualifications necessary for performing the core functions of this position.

Preferred Qualifications

Education:

Completion of an accredited licensed practical nurse program.

Experience:

Experience or clinical exposure in Post-Acute Care or Acute Care settings.

Licensure/Certification:

Current licensure in good standing as a Licensed Practical Nurse in Iowa.

Must maintain current Basic Life Support (BLS) certification or obtain it within three months of hire.

Valid driver's license required for work-related travel.

Knowledge/Skills/Abilities:

Proficient in reading, writing, and speaking fluent English.

Demonstrates multicultural sensitivity.

Basic computer skills, including Microsoft Office.

Patient-focused approach.

Strong critical thinking skills and independent judgment in decision-making.

Other:

Ability to use standard equipment necessary for performing essential job functions.

Occasional travel to other facilities may be required, using a UnityPoint Health vehicle, rental, or personal vehicle.
