The Younker 8 unit is a 23-bed Medical Surgical facility that offers a rich learning environment while providing care for patients with diverse diagnoses, particularly focusing on pulmonary conditions. Innovative Care Delivery Model
UnityPoint Health is implementing a forward-thinking care delivery model. The Collaborative Care approach allows all members of the multidisciplinary team to operate at the highest level of their expertise, utilizing their unique skills and training.
As an LPN within this model, you will be responsible for providing care to a designated group of patients, applying all the competencies acquired during your training. Supported by an RN and other team members, you will execute tasks within your scope of practice, which includes but is not limited to:
- Delivering safe and effective patient-centered care under the supervision of an RN.
- Collaborating with various team members.
- Conducting ongoing assessments and monitoring vital signs.
- Administering medications as prescribed by healthcare providers.
- Providing evidence-based patient education from the point of admission.
- Documenting daily progress towards care plan objectives.

Key Responsibilities
Clinical Excellence & Patient Care- Demonstrates the necessary knowledge and clinical skills to care for individuals with physical disabilities and chronic illnesses.
- Engages in patient and family education activities, utilizing appropriate resources to implement personalized teaching and discharge plans.
- Delivers hands-on nursing care, employing the nursing process to achieve optimal patient outcomes.
- Supports and reinforces care provided by other team members.
- Supervises unlicensed assistive personnel, applying clinical judgment within the LPN scope of practice, utilizing problem-solving techniques and effective time management.
- Coordinates nursing care activities in collaboration with interdisciplinary rehabilitation teams to achieve overall goals.
- Exhibits effective communication skills to build rapport with patients, families, and healthcare team members, ensuring compliance with legal documentation and reimbursement requirements.
- Maintains a strong commitment to the multidisciplinary healthcare team.
- Contributes to the assessment of patients' physical, psychological, sociocultural, and spiritual needs, as well as their educational and discharge requirements.
- Collaborates with rehabilitation team members to contribute to the dynamic interdisciplinary care plan.
- Implements the interdisciplinary care plan within the LPN scope of practice, providing nursing care and education directly or through ancillary personnel as needed.
- Participates in evaluating nursing care and modifying plans as necessary to meet measurable goals.
- Engages in interdisciplinary team processes during conferences and meetings, contributing to team decision-making within their scope of practice.
- Intervenes with team members and other healthcare professionals to ensure optimal recovery opportunities for patients.
- Collaborates with staff nurses and team members to provide cost-effective care, addressing emergent physical, psychosocial, and spiritual needs.
- Communicates effectively to facilitate group processes and team building.
- Advocates for and supports patients' and families' rights in care decisions.
- Practices in accordance with federal and state regulatory and accrediting agency standards.
- Reinforces information provided to patients regarding the underlying disease processes and teaches nursing techniques to help patients and families develop self-care skills.
- Assists patients and families in self-management and decision-making responsibilities, promoting independence and goal achievement.
- Supports the teaching conducted by staff nurses and other specialists, providing resource materials for patients' evolving needs.
- Prioritizes patient and workplace safety by preventing errors, accidents, and infections, and promptly reporting any concerns.

Education:
Completion of an accredited licensed practical nurse program.
Experience:
Clinical experience in Post-Acute Care or Acute Care settings.
Licensure/Certification:
Current licensure in good standing as a Licensed Practical Nurse in Iowa.
Basic Life Support (BLS) certification required within three months of hire.
Valid driver's license for work-related travel.
Knowledge/Skills/Abilities:
Fluent in reading, writing, and speaking English.
Culturally sensitive.
Basic proficiency in Microsoft Office.
Patient-focused approach.
Strong critical thinking skills and independent judgment in decision-making.
Other:
Familiarity with standard equipment used to perform essential job functions.
Occasional travel to other facilities may be required, utilizing a UnityPoint Health vehicle, rental, or personal vehicle.
