Job Summary:

As a Home Health Licensed Practical Nurse (LPN) for Faith, you will provide one-on-one skilled nursing care to patients using a comprehensive, patient-specific care plan set by the RN Case Manager. This plan establishes goals based on nursing diagnosis and incorporates preventative and rehabilitative nursing actions.

You will have the support and collaboration of a full team of healthcare professionals, working together to provide exceptional patient care.

Responsibilities:
  • Providing skilled nursing in the home or facility as ordered on the plan of treatment and nursing plan of care.
  • Assessing patients' condition, communicating, and properly documenting any pertinent information about the patients' conditions.
  • Promoting patient/family/caregiver education, taking into account the patient's needs and the standards of care.
  • Responsible for performing wound care, disease management, and medication reviews in the home setting as directed by physician and supervising RN.
  • Functioning as a team member by collaborating with the patient, physician, and interdisciplinary team.
Requirements:
  • Current, valid unencumbered Iowa State Board of Nursing LPN license.
  • Minimum of 1 year of home health experience.
  • Valid driver's license, reliable transportation, and auto insurance.
  • Current CPR certification.
