UnityPoint Health is implementing a groundbreaking care delivery model. The Collaborative Care approach allows all members of the multidisciplinary team to operate at the peak of their capabilities, utilizing your unique skills and training.
As an LPN within this model, you will deliver care to a designated group of patients, employing all the competencies acquired through your training. Supported by an RN and other team members, you will perform tasks within your scope of practice, including but not limited to:

- Works with team members to ensure optimal recovery opportunities for patients.
- Collaborates with staff nurses and team members to provide cost-effective care through appropriate clinical measures.

Clinical Excellence & Patient Education:
- Reinforces patient education regarding disease processes and teaches nursing techniques to empower patients and families in self-care.
- Assists patients and families in preparing for self-management and decision-making responsibilities, promoting independence and goal achievement.
- Supports the teaching provided by staff nurses and specialists, offering resource materials for evolving patient needs.
- Prioritizes patient and workplace safety by preventing errors and reporting concerns promptly.

Education:
Completion of an accredited licensed practical nurse program.
Experience:
Experience or clinical exposure in Post-Acute Care or Acute Care settings.
Licensure/Certification:
Current licensure in good standing as a Licensed Practical Nurse in Iowa. Basic Life Support (BLS) certification required within three months of hire.
Knowledge/Skills/Abilities:
Proficient in reading, writing, and speaking fluent English. Multicultural sensitivity. Basic computer skills in Microsoft Office. Customer-focused with critical thinking abilities.
Other:
Utilization of standard equipment necessary for fulfilling essential job functions. Occasional travel to other facilities may be required.
