Role Objective:

The Community Support Team is dedicated to recovery and resilience, providing intensive, community-based rehabilitation and outreach services. This collaborative team focuses on mental health rehabilitation interventions and support systems essential for assisting individuals in reaching and sustaining their recovery and resilience objectives. Services are delivered in natural environments and can be conducted face-to-face, via telephone, or through video conferencing with clients and their families/significant others, prioritizing the well-being and benefit of the individual.

Required Qualifications:

  • Expertise in individual, family, and group counseling, case management, and motivational interviewing
  • Demonstrated skills in crisis intervention and debriefing
  • Capability to organize and deliver educational sessions for clients, families, and community resources
  • A strong commitment to being part of a mission-driven organization
  • A collaborative approach to teamwork
  • A genuine desire to positively impact lives

Key Responsibilities:

  • Perform mental health and risk assessments on clients in crisis, both in the community and at designated locations.
  • Facilitate motivation and problem-solving discussions with staff regarding effective clinical practices.
  • Assume primary case management duties for assigned clients.
  • Conduct individual and/or family counseling and educational sessions for clients and their families.
  • Ensure that client service records are maintained in accordance with applicable standards, rules, and regulations.
  • Maintain communication with all staff regarding admissions, processes, and discharge plans.
  • Coordinate services from various community resources and organizational services.
  • Engage with law enforcement in diverse settings for daily staffing and follow-up communications.

Job Requirements:

  • Bachelor's degree in Counseling, Social Work, or a related field
  • Valid driver's license

Work Schedule: 8-hour shifts

  • Day shift, 9:30 AM - 6 PM
  • Monday to Friday
