Position Overview

CareStar, Inc. is seeking a dedicated Clinical Care Supervisor to lead our team of Care Coordinators. This role is essential in ensuring the delivery of high-quality care to our clients in the community.


Key Responsibilities
  • Supervise and mentor the Care Coordinator team, providing guidance and support in clinical and compliance matters.
  • Conduct ongoing evaluations of team performance, focusing on professional development and adherence to best practices.
  • Assist Care Coordinators with daily caseload management and crisis intervention as needed.
  • Ensure compliance with all relevant regulations and CareStar policies, maintaining confidentiality and integrity in all interactions.
  • Manage hiring, training, and performance evaluations for team members, fostering a culture of continuous improvement.
  • Stay informed about Medicare, Medicaid, and other relevant programs to provide accurate guidance to the team.

Qualifications

The ideal candidate will possess:

  • An active license as a Licensed Social Worker, Licensed Independent Social Worker, or Registered Nurse; or a Bachelor's Degree in Nursing, Social Services, or a related field with at least 2 years of relevant experience.
  • Strong problem-solving abilities and effective communication skills, both verbal and written.
  • Proficiency in Microsoft Office applications.
  • A valid driver's license and car insurance as required by state law.
