Care Coordinator Job Description

CareStar is seeking a highly motivated and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in conducting in-person eligibility evaluations for individuals applying for specific programs and creating person-centered service plans.

Key Responsibilities:
  • Conduct in-person eligibility evaluations for individuals applying for specific programs
  • Create person-centered service plans to assist children and their families in gaining access to needed services
  • Assist individuals and families in navigating the healthcare system and accessing medical, social, educational, and other resources
  • Develop and maintain relationships with community-based organizations and healthcare providers to ensure seamless care coordination
  • Utilize critical thinking and independent judgment to identify issues, develop solutions, and exercise judgment to meet requirements
Requirements:
  • Bachelor's Degree or Associate's Degree in social work, counseling, psychology, or a related field
  • Experience working with youth and their families who meet a functional need for behavioral healthcare
  • Knowledge of issues affecting individuals served and community-based interventions/resources for those individuals
  • Effective interpersonal (oral and written) skills to interact professionally and ethically with all age groups from a diverse population
  • Ability to utilize Microsoft Office programs and maintain confidentiality of patient information
