Ambulatory Care Coordinator

The Ambulatory Transitional Care Coach is a vital role within our organization, responsible for providing exceptional care coordination to patients recently discharged from an inpatient or observation hospital setting. This position plays a crucial part in ensuring a smooth transition of care, facilitating quality outcomes, and promoting patient satisfaction.
Key Responsibilities:- Provide outreach and care coordination to patients and their families, ensuring a seamless transition from hospital to home.
- Collaborate with healthcare teams to develop and implement personalized care plans, addressing the unique needs of each patient.
- Facilitate communication between patients, families, and healthcare providers, ensuring timely and effective care coordination.
- Identify and address potential barriers to care, such as medication adherence, follow-up appointments, and community resources.
- Monitor patient progress, making adjustments to care plans as needed to ensure optimal outcomes.
- High school diploma or equivalent required; degree in healthcare or related field preferred.
- Minimum 3 years of experience in a healthcare setting, with a focus on care coordination or patient transition.
