Job Summary

The Ambulatory Transitional Care Coach is a vital role within our healthcare team, responsible for providing exceptional care coordination and outreach services to patients recently discharged from an inpatient or observation hospital setting.

Key Responsibilities
  • Provide timely and effective care coordination to ensure seamless transitions of care
  • Collaborate with healthcare providers, patients, and families to develop and implement personalized care plans
  • Ensure accurate and efficient processing of patient referrals and scheduling of transition of care visits
  • Provide patient support and education on referrals and community resources to promote quality outcomes and smooth transitions
  • Act as a liaison between referral sources, facilities, and external entities to prevent and/or resolve continuum of care issues
Requirements
  • High School Diploma or equivalent
  • Three (3) years' experience in a healthcare setting
  • Proven customer service and patient interaction skills
  • Strong leadership and organizational abilities
  • Ability to communicate complex topics to diverse audiences
Preferred Qualifications
  • Medical Assistant or LPN experience
