Job Summary

UC Health is seeking a skilled Care Coordinator RN to facilitate and manage care coordination and case management across the continuum of care for our patients.

Key Responsibilities
  • Collaborate with healthcare teams to ensure effective and efficient coordination and management of care for patients.
  • Develop and implement individualized care plans to promote quality care and cost-effective interventions.
  • Maintain close contact with patients, families, care providers, payers, and community resources to ensure seamless care transitions.
  • Monitor and evaluate patient outcomes to identify areas for improvement and implement changes as needed.
  • Participate in multidisciplinary teams to share best practices and improve patient care.
Requirements
  • Bachelor of Science in Nursing degree.
  • 5 years of experience in care coordination or a related field.
  • Unencumbered Registered Nursing License for Ohio.
  • Demonstrated understanding of family-centered care and ability to work collaboratively.
  • Strong commitment to patient experience, service, and quality.
Preferred Qualifications
  • Master's degree in Nursing.
  • Demonstrated ability to work independently and make sound judgments.
  • Strong interpersonal, oral, and written communication skills.
