Hospice Liaison Coordinator

Job Summary: Hospice Of Virginia – an Addus family company is seeking a skilled Clinical Liaison to support Richmond and surrounding counties. This role involves effectively marketing our services, networking with healthcare professionals, and providing exceptional patient care.
About the Role:
- Represent and market company services at networking events and functions
- Assist with initial patient intake process, explaining benefits and providing skilled professional nursing evaluations
- Obtain data on physical, psychological, social, and spiritual factors impacting patients' needs and initiate intervention by interdisciplinary team members
- Schedule and coordinate educational programs for referral sources and community partners
What You'll Bring:
- Graduate from an accredited registered nursing program
- Licensed as a registered nurse in the state of practice
- Must possess current CPR and First Aid Certification
- 1 year experience preferred
- Able to make independent decisions and deal tactfully with patients and families
Compensation and Benefits:
- $85,000 - $100,000 per annum (based on location and experience)
