Job Description

This PRN opportunity offers weekday daylight hours and weekend availability, allowing for flexibility in your schedule. As a registered nurse, you will plan, organize, and direct hospice care, leveraging your experience in community health education. Your key responsibilities will include:

  • Completing comprehensive assessments to determine patient needs
  • Developing individualized care plans based on patient goals and nursing diagnosis
  • Providing palliative care and addressing pain/symptom management needs
  • Communicating with the physician and coordinating patient care with the interdisciplinary group

You will work closely with the family/caregiver and other team members to ensure continuity of care and meet the emotional needs of the patient and family. Additionally, you will participate in quality assessment performance improvement teams and activities.

Requirements and Qualifications

To be successful in this role, you must possess a graduate degree from an accredited school of nursing and current licensure as a Registered Nurse in your state. You should also have American Heart Association CPR certification or be willing to obtain it upon hire. A bachelor's degree is preferred, along with one year of hospice experience. Management experience is not required, but supervisory skills are essential for overseeing hospice aides.
