About the Role

We are seeking an experienced Registered Nurse to join our team as a Hospice Case Manager. In this impactful role, you will administer high-quality nursing care for patients of all ages in their place of residence.

Key Responsibilities
  • Coordinate care with the interdisciplinary hospice group, patient, family, and referring agency.
  • Develop and implement a plan of care to ensure quality and continuity of care.
  • Determine scope and frequency of services needed based on the acuity of your patient and their needs.
Requirements
  • A graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • A current registered nurse (RN) license in good standing.
  • At least two years of experience as a Registered Nurse in a clinical care setting, with experience in the hospice space highly preferred.

The estimated salary for this role is $65,000 - $85,000 per year, depending on experience and location. We offer a competitive benefits package, including reliable transportation and valid CPR certification requirements.
