Position Overview

The Community Support Specialist plays a crucial role in ensuring the safety and supervision of individuals with Intellectual Developmental Disabilities and behavioral health backgrounds. This position aims to promote healthy and socially acceptable behaviors while residing in community-based environments. The Specialist will collaborate with clinical teams, implement residential interventions, and assist clients in fulfilling their clinical objectives.

Key Responsibilities:

  • Supervise individuals in alignment with their person-centered Individual Support Plans (ISP), aiding them in achieving identified outcomes.
  • Maintain accurate documentation and administer medications as required.
  • Provide transportation for clients and support them in daily living activities, including meal preparation, personal hygiene, and toileting.
  • Facilitate community integration activities and assist clients in accessing community resources, such as medical appointments and social security services.
  • Complete additional tasks as assigned by the supervisor.

Qualifications:

  • A high school diploma or GED is mandatory; an associate's or bachelor's degree is preferred.
  • Basic computer skills and internet proficiency, with the ability to use applications for data entry and retrieval.
  • Previous experience in the human services sector is advantageous.
  • A valid driver's license is required.
  • Must pass employment screenings, including any necessary background checks.
  • Successful completion of a math assessment related to LLAM requirements.
