The Specialist will collaborate with clinical teams, implement residential interventions, and assist clients in fulfilling their clinical objectives.
Key Responsibilities:
- Supervise individuals in alignment with their person-centered Individual Support Plans (ISP), aiding them in achieving identified outcomes.
- Maintain accurate documentation and administer medications as required.
- Provide transportation for clients and support them in daily living activities, including meal preparation, personal hygiene, and toileting.
- Facilitate community integration activities and assist clients in accessing community resources, such as medical appointments and social security services.
- Complete additional tasks as assigned by the supervisor.
Qualifications:
- A high school diploma or GED is mandatory; an associate's or bachelor's degree is preferred.
- Basic computer skills and internet proficiency, with the ability to use applications for data entry and retrieval.
- Previous experience in the human services sector is advantageous.
- A valid driver's license is required.
- Must pass employment screenings, including any necessary background checks.
- Successful completion of a math assessment related to LLAM requirements.
