Clinical Nurse Coordinator

We are seeking a highly skilled and compassionate Registered Nurse - Clinical Nurse Coordinator to join our interdisciplinary care team. As a key member of our team, you will provide patient-centered care and be responsible for nursing assessment and diagnosis, planning, implementation, and evaluation.
Responsibilities- Conduct thorough nursing assessments to identify patients' physical, emotional, social, and spiritual needs.
- Develop and implement individualized plans of care based on patients' disease, symptoms, and response to treatment.
- Educate patients, families, and caregivers on disease processes, treatment options, and symptom management.
- Coordinate with the inter-disciplinary team to ensure continuity of care and effective communication.
- Refer patients, families, and caregivers to other clinical resources and professionals as needed.
- Implement, evaluate, and modify plans of care as necessary.
- Integrate research and evidence-based knowledge into clinical practice.
- Provide direct patient care, including education on chemotherapy, central lines, injections, and home care.
- Manage telephone and email symptom triage, dressing changes, medication administration, and injections.
- Conduct new patient orientations and provide caregiver support.
- Assess patients' needs for additional supportive care resources and coordinate outside clinical care.
- Collaborate with the research team for patients on trial and ensure continuity of care with outside providers.
- Graduate from an accredited school of nursing.
- Current Washington State Registered Nurse (RN) license or multistate RN license.
- Current BLS AHA Health Provider Card (or equivalent course).
- Critical thinking and ability to develop clinical judgment.
- Effective communication and interpersonal skills.
- Ability to work independently and as part of a team.
- BSN prepared nurse.
- OCN (Oncology Certified Nurse) or BMT RN.
- 2 years of oncology nursing experience.
- Previous telephone triage experience.
- Previous outpatient clinic experience.
