Fred Hutchinson Cancer Center (Fred Hutch) is seeking a skilled Clinical Nurse Coordinator to join our team. As a key member of our interdisciplinary care team, you will provide patient-centered care and be responsible for nursing assessment and diagnosis, planning, implementation, and evaluation.
Responsibilities:- Assess patients' physical, emotional, social, and spiritual needs and evaluate their adaptation to health changes.
- Develop a plan of care based on patients' disease, symptoms, and response to treatment.
- Educate patients, families, and caregivers on disease processes, treatment, side effects, and symptom management.
- Coordinate with the inter-disciplinary team to ensure continuity of care.
- Refer patients, families, and caregivers to other clinical resources and professionals as needed.
- Implement, evaluate, and modify the plan of care.
- Integrate research and evidence-based knowledge into clinical practice.
- Direct patient care, including education and support.
- Telephone and email symptom triage.
- Dressing changes, medication administration, and injections.
- New patient orientation and caregiver support.
- Assessment of needs for additional supportive care resources.
- Prescription refills and result reviews.
- Graduate from an accredited school of nursing.
- Current Washington State Registered Nurse (RN) license or multistate RN license.
- Current BLS AHA Health Provider Card (or equivalent course).
- Critical thinking and ability to develop clinical judgment.
- Time management skills and ability to communicate effectively.
- BSN prepared nurse.
- OCN (Oncology Certified Nurse) or BMT RN.
- 2 years of oncology nursing experience.
- Previous telephone triage experience.
- Previous outpatient clinic experience.
