Job Summary

We are seeking a highly skilled Clinical Program Coordinator to join our team at Swedish Health Services. This role will be responsible for the planning and coordination of the Swedish Aortic Center (SAC) program, ensuring seamless patient care and operational efficiency.

Key Responsibilities
  • Coordinate patient referral intake, follow-up, and care progression facilitation and tracking
  • Manage patient databases and track quality and operational metrics
  • Develop and implement program development initiatives, including education and outreach efforts
  • Collaborate with multidisciplinary teams to optimize patient, staff, and physician integration
  • Provide administrative support for the SAC program, including coordination of meetings and communication with stakeholders
Requirements
  • Bachelor's degree in Nursing or related field
  • Washington Registered Nurse License
  • 5 years of professional nursing experience in Cardiology, with demonstration of clinical judgment and leadership ability
