Position Overview:
Collaborate with the Centralized Follow Up and Patient Services Departments to design, execute, and manage centralized follow-up operations.

Enhance and restore patients' well-being by executing the nursing process; partnering with physicians and other healthcare professionals; offering physical and emotional support to patients and their families; overseeing assigned team members as necessary.


Key Responsibilities:

Centralized Laboratory Result Management:
Conduct lab result oversight as outlined by organizational Medical Standards and Guidelines (MSG) and Patient Services Procedures.

Effectively manage lab results, patient notifications, and follow-ups as dictated by approved plans established by Physicians and Clinicians based on organizational Medical Standards and Guidelines (MSG) and Patient Services Procedures.

Assist in the administration of the pre-natal program to ensure patients receive coordinated care. Provide educational resources regarding pre-natal care for patients.

Monitor and follow up on lab results and referrals, address missed appointments, track patient delivery outcomes, and ensure patients attend postpartum visits.

Maintain and update the pre-natal logbook and conduct quarterly audits as requested.
Receive, manage, and direct patient inquiries to the appropriate department for further assistance as necessary.
Deliver professional and thorough telephonic nursing assessments, evaluations, education, and guidance to patients in accordance with affiliate protocols.
Conduct post-surgical follow-up calls to patients within 24-48 hours post-procedure and complete necessary follow-up documentation.
Accurately document all patient communications.
Maintain current and precise electronic logs.
Notify pharmacies regarding new prescriptions and refill authorizations as directed by the provider.
Coordinate with contracted laboratories as needed.
Complete audits accurately and within specified timeframes as directed by the Director of Quality & Clinical Integration.
Provide exceptional and efficient telephone customer service to both internal and external clients.
Effectively communicate requests and respond accurately using established procedures for contacting clinics and/or clinic personnel.
Maintain a comprehensive understanding of the organization to address inquiries and direct calls appropriately.
Complete quality assurance monitoring tools as specified for the department.

Clinical Duties:
Provide nursing support at surgical sites as required and directed.

Assist in all facets of abortion care delivery for patients, including managing patient flow.

Offer counseling to patients undergoing surgical procedures. Provide oversight and guidance to health center assistants.
Foster a compassionate environment by delivering emotional and psychological support to patients and their families.

Ensure quality of care by adhering to organizational standards; evaluating health outcomes against patient care objectives and standards; making or recommending necessary adjustments; following care standards established by the state board of nursing, state nurse practice act, and other governing regulations.

Safeguard patients and staff by adhering to infection control policies and protocols, medication administration and storage procedures, and controlled substance regulations.

Document patient care services by charting in patient and departmental records.

Maintain a collaborative relationship among the healthcare team by sharing information; responding to requests; building rapport; and engaging in team problem-solving methods.


Qualifications:
Licensed Practical Nurse with a current license in good standing.
Must possess current CPR/BLS certification.
One year of experience in Women's Healthcare, abortion services, or recovery room preferred.
Bilingual English/Spanish skills preferred.

Must be willing to orient in surgical and family planning environments to gain an understanding of clinic operations and must be willing to cover surgical shifts as needed.

Ability to utilize a computer for charting, documentation, and communication.
Ability to communicate effectively with clients from diverse backgrounds.
Ability to proactively engage in team development and problem-solving.
Excellent verbal, written, and interpersonal communication skills.
Ability to manage multiple tasks simultaneously and thrive in a fast-paced environment.
Ability to work independently within established protocols.
Comfortable working in a paperless environment, including the electronic management of all patient information.
Willingness to work flexible hours, including evenings and weekends.
Ability to maintain strict confidentiality and demonstrate a thorough understanding of HIPAA and confidentiality guidelines.
Effectively master the use of electronic health records and electronic practice management systems to enhance efficiency in lab result management and documentation.
