Job Overview

Position Summary: Collaborate with Centralized Follow Up and Patient Services Departments to design, execute, and manage centralized follow-up and patient navigation processes. Enhance and support patients' well-being by executing the nursing process; working alongside physicians and other healthcare professionals; offering both physical and emotional support to patients and their families; and providing language interpretation services as necessary. Additionally, this role will contribute to the affiliate Centering Pregnancy Program and take on specific duties as a Patient Navigator.

Key Responsibilities:

Centralized Follow-up Duties:

  • Manage laboratory result processes as outlined by established Medical Standards and Guidelines.
  • Effectively handle lab results, patient notifications, and follow-ups according to approved plans.
  • Deliver professional telephonic nursing assessments, evaluations, education, and guidance to patients in line with affiliate protocols.
  • Ensure accurate documentation of all patient communications.
  • Notify pharmacies regarding new prescriptions and refill authorizations as directed by healthcare providers.
  • Coordinate with contracted laboratories as necessary.
  • Maintain a comprehensive understanding of the organization to address inquiries and direct calls appropriately.
  • Complete quality assurance monitoring tools as required by the department.

Clinical Duties:

  • Provide nursing support at surgical sites as needed.
  • Assist in delivering abortion care, ensuring patient support and throughput. Offer counseling for patients undergoing surgical procedures and supervise healthcare assistants.
  • Ensure care quality by adhering to established standards; evaluate health outcomes against care goals; recommend necessary adjustments; and comply with state nursing regulations.
  • Safeguard patients and staff by following infection control policies and medication protocols.
  • Document patient care services accurately in departmental records.

Centering Pregnancy Program Duties:

  • Independently oversee the Centering Pregnancy program, including:
  • Creating inclusive prenatal education materials for diverse and high-risk populations.
  • Planning, organizing, and facilitating group meetings.
  • Identifying and reaching out to prenatal patients for program participation.
  • Managing resources and evaluating program effectiveness.
  • Providing individual counseling on various aspects of prenatal health and connecting patients to community resources.
  • Monitoring prenatal patient lists to ensure appointment adherence and follow-up.
  • Coordinating with prenatal providers to enhance care coordination.

Patient Navigator Duties:

  • Organize and manage patient appointments to ensure timely access to services.
  • Facilitate communication between patients, support persons, and healthcare providers to enhance satisfaction and care quality.
  • Inform patients about financial aspects of care and connect them to financial resources.
  • Assist patients in identifying logistical needs and linking them to appropriate resources.
  • Provide interpretive language services as required.
  • Collect and report necessary program data.
  • Identify and address gaps in patient navigation processes to ensure effective support for high-need patients.
  • Collaborate with external vendors to facilitate access to essential services.
  • Work closely with affiliate staff to improve workflows and enhance patient experiences.
  • Document patient interactions, progress, and outcomes while maintaining confidentiality in accordance with policies.

Qualifications:

  • Licensed Practical Nurse with a valid NYS license.
  • Current CPR/BLS certification.
  • 1-2 years of experience in Women's Healthcare or abortion services preferred.
  • Strong public speaking and facilitation skills.
  • Ability to thrive in a fast-paced environment and work independently.
  • Excellent organizational and time management skills.
  • Bilingual in Spanish/English preferred.
