Job Summary

We are seeking a highly skilled and compassionate Care Manager to join our team at Superior HealthPlan. As a Care Manager, you will play a critical role in developing and implementing personalized care plans for our long-term care members, ensuring they receive the highest quality of care and support.

Key Responsibilities
  • Care Planning and Coordination: Develop and implement comprehensive care plans that address the unique needs and goals of each member, in collaboration with healthcare providers and community resources.
  • Member Engagement and Education: Educate members and their families/caregivers on available services, benefits, and resources, empowering them to make informed decisions about their care.
  • Collaboration and Communication: Foster strong relationships with healthcare providers, community organizations, and other stakeholders to ensure seamless care coordination and effective communication.
  • Case Management and Monitoring: Monitor member progress, identify potential issues, and make recommendations for adjustments to care plans as needed.
  • Documentation and Reporting: Maintain accurate and detailed records of member interactions, care plans, and outcomes, ensuring compliance with regulatory requirements.
Requirements
  • Education: Bachelor's degree in a related field, such as social work, nursing, or healthcare administration.
  • Experience: 2-4 years of experience in care management, case management, or a related field.
  • Licensure/Certification: LPN, LVN, LBSW, LMSW, LCSW, LPC, or RN preferred, but not required.
Preferred Qualifications
  • Advanced Education: Master's degree in a related field.
  • Specialized Experience: 2+ years of experience working with individuals with intellectual and developmental disabilities or related conditions.
  • Strong Communication and Interpersonal Skills: Excellent communication and conflict resolution skills, with the ability to build strong relationships with members, families/caregivers, and healthcare providers.
  • Organizational and Time Management Skills: Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
