Client Support Specialist
Full Time
Salary Range: $20.25 To $20.25 Hourly
Schedule: 40 hours/week, Flexible; Monday to Friday, 8 am – 6 pm

Key Responsibilities:
- Facilitate the planning and scheduling of annual reviews. Assist clients in developing personalized care plans and objectives based on assessments and individual aspirations.
- Conduct regular meetings with clients, ensuring consistent communication with family members and team professionals as necessary.
- Provide guidance on nutrition through meal planning, shopping lists, and kitchen safety.
- Support clients in managing their finances, including budgeting and bill payments, while collaborating with relevant parties.
- Assist clients in maintaining their living spaces and ensuring home safety.
- Promote personal hygiene and develop tailored plans when necessary, including coordination with health professionals.
- Coordinate health care services with clients, guardians, and medical professionals, ensuring quality care standards are met.
- Facilitate transportation for clients as needed.
- Encourage effective communication and social skills among clients, aiding in the development of healthy relationships.
- Advocate for clients to ensure they receive timely and appropriate services.
- Collaborate with various agencies to coordinate services, ensuring accuracy and timeliness.
- Utilize organizational tools to maintain accurate case records and documentation as required.
Qualifications:
A Bachelor's Degree in a relevant field is preferred, along with two years of experience working with adults with cognitive or social disabilities.
Essential Skills:
- Cultural awareness and ability to engage with diverse populations.
- Strong verbal and written communication skills.
- Proficiency in computer applications, including Outlook, Excel, and Word.
- Excellent organizational abilities and a commitment to punctuality.
- Valid driver's license.
