Job Summary

We are seeking a skilled Clinician to join our team at Vermont Employer. The successful candidate will provide whole-person care coordination and support to individuals and the SASH population as a whole.

Key Responsibilities
  • Provide direct individual therapeutic support through in-person, phone, and telehealth sessions, including individual, couples, and family therapy, and other psychosocial support.
  • Assess, screen, and triage services to identify unaddressed needs and connect individuals with clinician and/or community resources and providers.
  • Develop and lead group-based programs on topics related to mental health, substance use, aging, and emotional needs.
  • Collaborate with SASH staff, primary care, psychiatric, and other community partners and service providers to ensure comprehensive care.
  • Provide training and consultation to SASH Coordinators and Wellness Nurses on challenges faced by SASH participants with mental illness, substance misuse, and other social, behavioral, or emotional concerns.
  • Utilize SASH data management systems to collect and present outcome data for the program.
Requirements
  • Masters Degree in counseling, social work, psychology, or a closely related field.
  • Licensed or on the roster of Non-licensed Non-certified Psychotherapists in Vermont.
  • Valid Driver's license and use of a personal vehicle.
