Job Summary

We are seeking a highly skilled Clinical Care Coordinator to join our team at United Counseling Services. The successful candidate will provide whole-person care coordination and support to individuals and communities, working closely with our SASH team and community partners.

Key Responsibilities
  • Direct Service: Provide individual therapeutic support, assessment, and triage services to help identify unaddressed needs and connect individuals to clinician and/or other community resources and providers.
  • Care Coordination: Collaborate with embedded SASH staff, participate in regular consulting meetings, and outreach to primary care, psychiatric, and other community partners and service providers.
  • Consultation and Training: Provide training and consultation to SASH Coordinators and Wellness Nurses on challenges confronted by SASH participants with mental illness, substance misuse, and other social, behavioral, or emotional concerns.
  • Administrative: Utilize SASH data management systems to collect and present outcome data for the program, accurately complete timesheets and expense reimbursements, and perform other administrative tasks as required.
Requirements
  • Education: Master's Degree in counseling, social work, psychology, or closely related field.
  • Licensure: Licensed or on the roster of Non-licensed Non-certified Psychotherapists in Vermont.
  • Other: Valid Driver's license and use of a personal vehicle.
