ROLE RESPONSIBILITIES:

The Quality Improvement Coordinator plays a crucial role in overseeing quality improvement reviews and coding in partnership with Quality Improvement Specialists. This position is tasked with monitoring quality metrics, identifying areas for enhancement, and collaborating with the QI Team to ensure compliance and educational initiatives. Key responsibilities include:

  • Facilitating OASIS time-point submissions for evaluation with QI Specialists and Medical Coders, ensuring effective communication with clinicians and management regarding compliance issues.
  • Ensuring timely coding of Home Care and Hospice records, assisting in obtaining necessary documentation from patients' primary care providers and referral sources as required.
  • Assigning new patient records to appropriate teams and payors.
  • Coding various patient records in accordance with ICD-10 coding standards as directed by the supervisor.
  • Responding promptly to requests for information from Medicare and Medicaid, collaborating with the Utilization Review Technician and Billing Team.
  • Maintaining accurate files and generating periodic reports on clinical data and trends, including LUPA's, NTU, and clinician submissions.
  • Running reports for Homecare and Hospice as needed, including LUPA, ACH, Clinical Scorecard, and participating in record audits.
  • Staying informed on regulatory changes affecting OASIS and ICD-10 coding from both State and Federal levels.
  • Coordinating medical records requests in line with Agency and HIPAA regulations.
  • Conducting patient care calls based on criteria set by the Director of Quality to enhance patient satisfaction and outcomes, logging and reporting these calls regularly.
  • Ensuring corrections to OASIS time-points are made and that all necessary documentation is present in patient records, preparing care plans as needed.
  • Verifying the completeness of F2F documents per CMS guidelines and assisting in obtaining any missing documentation.

QUALIFICATIONS:

  • High School diploma or equivalent is required.
  • Experience in a home health agency is preferred.
  • OASIS Certification is preferred.
  • Medical Coding Certification is preferred.
  • A valid state driver's license is required, along with the ability to pass background checks.
