Job Summary

We are seeking a highly skilled and compassionate Social Worker to join our team. As a Social Worker at Suncrest Hospice, you will play a vital role in supporting patients and their families navigate the complexities of end-of-life care. Your expertise will help us deliver personalized care, addressing the physical, emotional, and social needs of our patients.

Key Responsibilities

  • Assess and address the psychosocial needs of patients and their families, providing guidance and support to ensure optimal well-being.
  • Develop and implement individualized care plans, working collaboratively with interdisciplinary teams to ensure seamless care coordination.
  • Provide counseling and education to patients and families, empowering them to make informed decisions about their care.
  • Connect patients and families with community resources, advocating for their needs and ensuring access to necessary support services.
  • Maintain accurate and comprehensive records, documenting patient interactions, progress, and outcomes.
  • Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices.

Requirements

  • Master's degree in Social Work (MSW) from an accredited institution.
  • Proven experience in social work, preferably in a hospice or palliative care setting.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare professionals.
  • Strong assessment and problem-solving skills, with the ability to think critically and make sound judgments.
  • Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
