About the Role: Financial Liaison

Position Summary: The Financial Liaison plays a crucial role in assessing and determining the financial eligibility of prospective residents and those transitioning from rehabilitation to long-term care within a skilled nursing environment. Key responsibilities include verifying insurance coverage, obtaining necessary authorizations, and engaging with families, legal representatives, case managers, and financial advisors regarding insurance matters, assets, transfers, and payment options including Medicaid.

Core Responsibilities:

  • Collaborate with Admissions, Business Office, and families to analyze and anticipate payer status, identifying potential challenges for new admissions, re-admissions, and current residents.
  • Conduct insurance verifications for all new admissions and re-admissions, securing pre-authorizations as required; relay pertinent information to staff, families, and residents regarding coverage status.
  • Arrange consultations with newly admitted residents and/or families to address inquiries about Medicare, Medicaid, private payment options, and insurance plans.
  • Issue denial letters promptly to families and communicate relevant information to other departments as necessary.
  • Review and finalize financial documentation and Admissions Agreements with families or their financial representatives.
  • Assess applications for long-term care from rehabilitation residents as forwarded by Rehabilitation Social Workers.
  • Allocate residents to rehabilitation and long-term care beds in accordance with payer allowances.
  • Organize appointments and facility tours for prospective residents and their families.
  • Support new admissions by coordinating room readiness, collaborating with building services, environmental services, and neighborhood teams.
  • Responsible for creating and maintaining resident records, ensuring accurate insurance information is updated in compliance with audit standards, including bed hold statuses for hospitalized residents or those on social leave.
  • Assist in tracking rejected or withdrawn referrals; maintain a database regarding the status of all referrals.
  • Oversee the maintenance of fax machines, monitor supplies, and liaise with vendors as needed.
  • Foster relationships with assigned neighborhoods to ensure ongoing communication regarding census updates and operational highlights.

Qualifications:

  • Bachelor's Degree in Social Work, Human Services, or a related discipline.
  • A minimum of three years' experience in skilled nursing facilities, with a solid understanding of the admissions process, census management, and payer mix.
  • Proficient knowledge of Medicaid, Medicare, private payment, and third-party insurance processes.
  • Ability to prioritize and manage multiple tasks effectively in a dynamic office environment.

Physical Requirements:

  • Occasional assistance with moving elders.
