The Manager, Patient Care Coordination plays a crucial role in integrating and overseeing utilization management, care coordination, discharge planning, and performance enhancement activities for all care coordinators. This position is responsible for managing the caseloads of designated team members, ensuring effective staffing, optimizing resource utilization, and maintaining compliance with evidence-based practices and regulatory standards.

Key Responsibilities

A typical day for a Manager, Patient Care Coordination at Hackensack Meridian Health involves:

  • Facilitating collaborative patient care management across various stages, addressing barriers to timely and effective care delivery and reimbursement.
  • Overseeing staff involved in care management processes, including length of stay, patient throughput, and follow-up on denials and appeals.
  • Implementing process improvement strategies to assess team members' documentation.
  • Ensuring the execution of multidisciplinary rounds and team meetings.
  • Providing guidance to the multidisciplinary team in complex cases.
  • Collecting, analyzing, and presenting metrics related to care management.
  • Participating in key meetings to discuss existing case management processes.
  • Developing performance improvement plans for care coordinators, documenting performance, and providing constructive feedback and recognition for effective performance.
  • Identifying opportunities for improvement and recommending process enhancements.
  • Adhering to HR policies regarding performance management and disciplinary actions when necessary.
  • Engaging in departmental preparations for regulatory inspections and compliance audits.
  • Coordinating patient care progression by collaborating with the multidisciplinary team.
  • Working with ancillary departments to ensure the accuracy of patient demographic and insurance information.
  • Assisting in the collection and analysis of indicators that track the efficiency of case management processes.
  • Utilizing data to inform decisions and implement performance improvement strategies related to assigned staff, including fiscal, clinical, and patient satisfaction metrics.
  • Collaborating with Physician Advisors and Hospitalists on case management needs and complex cases.
  • Maintaining annual competencies and ensuring ongoing training and education for the team in relevant platforms.
  • Assuming supervisory responsibilities for other care coordination managers in the absence of a Supervisor.
  • Performing additional duties and projects as assigned.
  • Complying with organizational competencies and standards of behavior.

Qualifications

Required Education, Knowledge, Skills, and Abilities:

  • Bachelor's degree in Nursing or a Master's degree in Social Work.
  • A minimum of 3 years of full-time experience in an acute care environment.
  • Familiarity with hospital and community resources, as well as resource/utilization management.
  • Experience in care coordination, case management, or discharge planning.
  • Strong decision-making and problem-solving abilities, with a creative approach to challenges and influential leadership skills.
  • Excellent verbal, written, and presentation capabilities.
  • Moderate to advanced computer proficiency.
  • Proficient in Microsoft Office and/or Google Suite platforms.

Licenses and Certifications Required:

  • New Jersey State Professional Registered Nurse License or New Jersey Licensed Social Worker or New Jersey Licensed Clinical Social Worker.

Preferred Licenses and Certifications:

  • Basic Cardiac Life Support Certificate.
