About the Role

We are seeking a highly motivated and compassionate Community Health Worker to join our team at UnitedHealthcare. As a Community Health Worker, you will play a vital role in helping our members navigate the healthcare system and achieve their health goals.

Key Responsibilities
  • Assessment and Planning: Conduct thorough assessments of members' health needs and develop personalized care plans to address their unique challenges.
  • Resource Navigation: Utilize company and community-based resources to establish safe and effective case management plans for members.
  • Collaboration and Communication: Work closely with members, families, and healthcare providers to ensure seamless care coordination and communication.
  • Referral and Advocacy: Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services, and advocate for members' needs and choices.
  • Documentation and Reporting: Accurately document all member assessments, care plans, and referrals provided.
Requirements
  • Education: Bachelor's Degree in a health-related field or social work, or Active Licensed Practical Nurse (LPN) license.
  • Experience: 1+ years of field-based experience and 1+ years of knowledge of community resources, culture, and values.
  • Skills: Intermediate level of computer proficiency, including Microsoft Outlook and Teams, and ability to use multiple web applications.
  • Travel: Valid Driver's license, current automobile insurance, and access to reliable transportation for local travel up to 100 miles round trip and up to 50% of the time.
Preferred Qualifications
  • Community Health Worker (CHW) Accreditation
  • Experience working in Managed Care
  • Knowledge of Medicaid/Medicare population
