Patient Care Coordinator

The Patient Navigator plays a vital role in ensuring seamless clinical coordination and patient access to healthcare services. As a liaison between patients, departments, and care providers, this individual will facilitate timely appointments, coordinate care, and provide education to patients and their families.
Key Responsibilities- Coordinate patient navigation: Assist patients in understanding their diagnosis, treatment options, and available resources, including clinical research studies and technologies.
- Coordinate between patients, departments, and care providers: Serve as a liaison between patients and all other care providers, departments, and vendors, ensuring optimal clinical coordination.
- Scheduling and coordination of care: Facilitate timely appointments for consults and support services within established service standards.
- Patient education: Schedule education sessions in person or by virtual visits, as appropriate, with patients and clinical staff, and document all communication and education in the patient's medical record.
- Maintenance of clinical/patient data: Maintain collection and storage of data and clinical information specific to the service, including filing and uploading clinical documentation into the Electronic Health Record (EHR) and electronic programs.
- Required: High School Diploma/GED or equivalent and 3 years of patient navigation, scheduling, or outpatient administrative support services.
- Ability to navigate EPIC-Electronic Health Records software.
- Ability to speak concisely and effectively communicate.
- Self-motivated.
- Ability to prioritize work to meet deadlines.
- Ability to function in a high-pressure environment and meet stringent deadlines for completing tasks.
- Ability to handle multiple tasks simultaneously.
- Strong customer service and patient relations skills.
- Knowledge of Microsoft Office.
