About the Role:

As a Care Coordinator, you'll play a vital role in facilitating and coordinating patient care plans to ensure quality and continuity. You'll collaborate with our interdisciplinary team to implement care plans, transition strategies, and patient education, all with the goal of achieving desirable patient outcomes and efficient resource utilization.

Key Responsibilities:
  • Develop individualized care plans that consider patient and caregiver goals, preferences, and desired level of involvement.
  • Make outbound calls to assess patient health status and identify gaps or barriers in treatment plans.
  • Coordinate care for patients, making referrals to outside sources as needed.
  • Ensure patients discharging from hospital or emergency department receive necessary services and resources, including medication reconciliation.
  • Triage symptom-based calls per approved protocols for a wide range of patients.
  • Enroll patients in home monitoring programs and provide education on chronic and acute disease processes.
  • Document all workflows in our EPIC system for tracking and quality assessment.
  • Provide patient education to assist with self-management of wellness and disease.
  • Form partnerships with providers to achieve quality and cost management objectives.
  • Track and monitor enrolled patients throughout the continuum of care.

Requirements:
  • LA CNA Certification required.
  • Adult First Aid & CPR Certification required.
  • Memory care experience preferred.
  • Experience working with elderly preferred.
