Job Overview:
The Special Needs Program Coordinator plays a critical role in delivering high-quality patient care services to our enrollees. As a skilled healthcare professional, you will work collaboratively with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.

Responsibilities:
* Coordinate care transitions for patients with complex needs
* Develop and maintain relationships with internal and external stakeholders, including community agencies and providers
* Collaborate with case managers to ensure continuity of care and quality outcomes
* Analyze data to identify trends and areas for improvement in clinical operations

Requirements:
* Bachelor's degree in a related field (e.g., healthcare administration, nursing, social work)
* 2-3 years of experience in case management or a related field
* Strong communication and interpersonal skills

Salary Range: $65,000 - $90,000 per year (depending on experience)
