Deliver essential services and resources to individuals and families aimed at fostering rehabilitation, recovery, and resilience.
Recognize and leverage the strengths that will assist individuals or families in their recovery journey while addressing obstacles that may hinder the development of skills necessary for independent community living.
Provide support to individuals and families during crises; implement tailored interventions to enhance an individual's capacity to make informed and autonomous decisions.

Ensure compliance with hospital and departmental policies and procedures.
Patient care responsibilities may encompass Pediatric, Adolescent, Adult, and Geriatric populations.
Key Responsibilities:
- EDUCATION - Foster and support the acquisition of skills in daily living activities, interpersonal coping, socialization, and community engagement.
- PATIENT CARE - Assist in securing financial and medical benefits; promote the maintenance of safe, affordable, and stable housing.
- PATIENT CARE - Aid in identifying and resolving developmental, familial, social, educational, and work-readiness challenges.
- PATIENT CARE - Provide information and resources regarding mental health conditions; act as a mentor to encourage recovery and resilience, instilling hope.
- EDUCATION - Instruct on symptom monitoring, management, and relapse prevention; offer insights into co-occurring disorders.
- EDUCATION - Conduct skills training sessions for clients.
- PATIENT CARE - Help individuals gain a comprehensive understanding of their medications and address any concerns regarding their treatment plans.
- PATIENT CARE - Support the development of self-motivation skills for adhering to prescribed medication regimens; encourage individuals to recognize potential symptoms that may arise from non-compliance.
- SERVICE PLAN - Collaborate in creating and coordinating the individual's service plan based on identified strengths and goals, including recovery/resilience management and crisis intervention strategies.
- COORDINATION - Assist in coordinating services and resources to ensure individuals access necessary rehabilitative, medical, and ancillary services.
- SUPPORT SERVICES - Promote the establishment and eventual independence of natural support systems within workplace, educational, familial, and community settings.
- ASSESSMENT - Partner with individuals and families to identify personal strengths, needs, and barriers to achieving self-defined goals. Conduct ongoing evaluations to ensure services meet the individual's requirements.
- CRISIS INTERVENTION - Evaluate, support, and intervene during crises, utilizing individualized crisis management plans that recognize early warning signs of crisis or relapse and promote the use of natural supports. Advocate for alternatives to emergency department visits and inpatient care.
- DOCUMENTATION - Maintain comprehensive records of all client interactions and those conducted on their behalf; complete and submit necessary billing documentation; ensure client files are current and thorough.
- STATISTICS - Compile and report relevant statistics regarding programs and client services.
- DEVELOPMENT - Pursue professional growth through participation in educational programs, reviewing current literature, and attending workshops and meetings.
- PATIENT SAFETY - Adhere to policies, procedures, and protocols related to patient safety.
- PATIENT SAFETY - Proactively seek opportunities to enhance patient safety by questioning existing policies and processes.
- PATIENT SAFETY - Identify and rectify environmental conditions that may pose risks to patient safety.
- PATIENT SAFETY - Timely report any potential or actual patient safety issues, medical errors, or near misses.

Qualifications:
Essential:
* High School Diploma or GED Equivalent
Experience:
Essential: Nonessential: Bilingual in English and Keres, Tewa, Tiwa, Towa, Zuni, or Navajo
Credentials:
- CPR for Healthcare/BLS Provider or Professional Rescuers within 30 days
- Certified Peer Specialist recognized in the State
- Valid Driver's License
- Vehicle Operator's Permit within 60 days
Physical Requirements:
Medium Work:
Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
Physical demand requirements exceed those for Light Work.Working Conditions:
