Job Summary

The Community Peer Support Worker is a critical role at State of the Heart Recovery Inc, responsible for providing intensive case management services to individuals with co-occurring mental health and substance abuse disorders. This position requires a strong understanding of the recovery process and a passion for supporting others in their journey.

Responsibilities and Duties
  • Develop and implement treatment plans in collaboration with clients and other staff members.
  • Provide crisis intervention and relapse prevention support, as well as emergency services as needed.
  • Conduct ongoing psychosocial assessments and provide family support services.
  • Support clients with vocational needs, including resume building and interview skill development.
  • Facilitate groups on topics such as housing needs, vocational skills, and life skill building.
  • Document all interactions in a timely and accurate manner, adhering to state licensing and federal regulations.
  • Identify care coordination needs and assist in linking clients with a care coordinator.
  • Work cooperatively with other community resources and referral sources to coordinate services.
Qualifications
  • Active Certified Peer Support Worker (CPSW) license.
  • High school diploma or equivalent, with a self-identified history of mental health or substance abuse services.
  • Minimum of 3 years in recovery.
Skills
  • Motivation to support others in the recovery process.
  • Sensitivity to individual needs and respect for personal independence and ability.
  • Experience with Medications for Addiction Treatment (MAT).
  • Basic computer skills and software proficiency.
  • Effective de-escalation techniques and Motivational Interviewing skills.
Additional Requirements
  • Active New Mexico Medicaid number or obtain within 90 days of employment.
  • Own vehicle and auto insurance for transporting clients.
  • Cleared criminal background check and signed statement regarding felony indictments and/or convictions.
  • Cleared TB test results within 12 months prior to employment.
  • Cleared pre-employment drug screen.
  • First Aid and CPR certifications within 90 days of employment and maintained thereafter.
