Care Coordinator Assistant Opportunity

United Homecare Services, a leading non-profit organization, seeks a highly skilled Care Coordinator Assistant to join our team in Beaverton, Oregon. As a key member of our care coordination team, you will play a vital role in ensuring the delivery of exceptional care services to our clients.

Responsibilities:
  • Maintain professional communications with clients, caregivers, and internal teams via email, phone, and face-to-face interactions.
  • Answer phone inquiries and provide timely information to clients and caregivers.
  • Conduct in-home consultations and assessments to understand client needs and preferences.
  • Manage client and caregiver schedules in our ClearCare database, ensuring seamless coordination and communication.
  • Assign caregivers to clients based on their skills and needs, and facilitate ongoing training and support.
  • Evaluate client service plans and update them as needed to ensure optimal care outcomes.
  • Be available by phone 24/7 while on-call and participate in on-call rotations.
  • Perform caregiver duties in emergency situations with short notice, and travel within our 60-mile service area to provide care services.
  • Maintain accurate and up-to-date records, including electronic data, client files, and employee files.
  • Ensure compliance with agency policies and procedures, and perform other office duties as needed.
