We are seeking a highly skilled and compassionate MDS Coordinator RN to join our team at Beaverton Rehab & Specialty Care LLC. As a key member of our healthcare team, you will be responsible for coordinating patient care, ensuring accurate and timely documentation, and promoting a positive and supportive environment for our residents.
Responsibilities- Coordinate patient care plans, ensuring accurate and timely documentation and implementation.
- Collaborate with healthcare team members to ensure seamless care delivery and effective communication.
- Monitor and analyze patient data, identifying trends and areas for improvement.
- Develop and implement strategies to improve patient outcomes and satisfaction.
- Provide education and support to patients, families, and caregivers on healthcare-related topics.
- Current RN licensure in the state of Oregon.
- Minimum 2 years of experience in a healthcare setting, preferably in a long-term care or rehabilitation environment.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to analyze data and identify trends, with a focus on improving patient outcomes.
- Strong problem-solving and critical thinking skills, with the ability to prioritize tasks and manage multiple responsibilities.
