RN Care Coordinator Team Lead

Key Responsibilities:
- Practices clinical and leadership skills, evaluating and prioritizing staffing needs while collaborating with managers and department assistants.
- Collaborates with clinicians, advocates for patients, and identifies scope, workflow, and policy/procedure involvement.
- Mentors and coaches team members on patient care and workflow issues, demonstrating effective communication and critical thinking.
- Participates in the hiring, training, and evaluating process, facilitating orientation and training needs.
- Assesses and monitors clinical competencies, provides input into performance evaluations, and acts as a mentor and coach for team members.
- Coordinates communication flow within the team and other departments, participating in local and regional issue communication and facilitating information flow during emergency situations.
- Minimum two years of skilled nursing, LTAC, or acute rehabilitation experience.
- Minimum one year of medical, surgical, orthopedic, LTAC, acute rehabilitation, or critical care nursing experience in an acute setting.
- BSN or bachelor's degree in a healthcare-related field or currently enrolled in a BSN program, or four years of directly related experience.
- High School Diploma or General Education Development (GED) required.
- Registered Nurse License (Washington) within six months of hire or Compact License: Registered Nurse within six months of hire.
- Registered Nurse License (Oregon) within six months of hire.
- Basic Life Support within three months of hire.
- Minimum three years of Medical/Surgical, Critical Care, or NICU Nursing.
- Minimum three years of recent experience in one or more of the following areas: Hospital Inpatient Discharge Planning, SNF Placement, Skilled nursing, LTAC, Acute Rehabilitation, Community Health, Home Health, and/or Utilization Management.
- Certified Case Management (CCM) is preferred upon hire/transfer.
- Knowledge of appropriate utilization of acute hospital and Kaiser Permanente internal resources.
- Knowledge of Medicare and Medicaid regulations related to eligibility requirements: hospital, nursing facilities.
