Position Overview:

Contingent Pharmacist

Afternoons/Rotating Weekends (every other weekend or every 3rd weekend)

Under the supervision of the Manager of Community Pharmacy Services and the Manager of Inpatient Pharmacy Services, along with the Clinical Pharmacy Manager, the incumbent will deliver clinical support and distribution services (pharmaceutical care) in collaboration with the healthcare team to ensure high-quality, safe, and cost-effective medication therapy for patients.

Qualifications:

Minimum Education, Licensure / Certification and Experience Required:

A. Education
  • A Bachelor of Science in Pharmacy from an accredited College of Pharmacy or equivalent is the minimum acceptable educational requirement.
  • A Doctor of Pharmacy degree and/or additional ACCP accredited certification is highly preferred.
B. Licensure / Certification
  • Must be a registered pharmacist with eligibility for a Michigan Pharmacist and Controlled Substance license.
  • Current Basic Life Support (BLS) certification is required.
  • If the pharmacist is part of the Rapid Response Team or responds to Codes, Advanced Cardiac Life Support (ACLS) certification is mandatory.

  • Exceptional customer service skills necessary for effective interaction with various levels of hospital personnel, external customers, and community groups.
D. Experience
  • Prior experience in hospital, retail, or long-term care settings based on departmental function area.
Key Responsibilities:
  • Interprets and assesses all physicians' medication orders to ensure suitable drug therapy for each patient in compliance with legal and regulatory standards.
  • Reviews, maintains, and evaluates patient medication orders and profiles for allergies, drug-drug interactions, drug-food interactions, and therapeutic duplications.
  • Engages with physicians, nurses, and other healthcare providers to clarify orders that are ambiguous, incomplete, or inappropriate, and recommends alternative therapies as necessary. Document all intervention activities appropriately.
  • Delivers drug and poison information education programs to healthcare professionals in response to inquiries or during in-service training.
  • Accurately prepares and dispenses medications and IV admixtures for both inpatients and outpatients.
  • Compounds and dispenses medications and injectables, ensuring that competency is demonstrated before engaging in compounding or verifying compounded products.
  • Adheres to USP 797/800 Guidelines.
  • Supervises pharmacy technicians and interns in distribution functions and departmental operations, contributing to annual performance evaluations.
  • Facilitates the training and orientation of new pharmacists, pharmacy technicians, and pharmacy interns.
  • Maintains accountability for the management of Controlled Substances, hazardous drugs, and investigational drugs in accordance with Federal and State regulations, ensuring proper record-keeping.
  • Participates in pharmaceutical care activities, including patient rounds, therapeutic drug monitoring, patient education, and pain management as appropriate.
  • Makes informed decisions promptly to optimize drug costs while ensuring the efficacy and safety of medication therapy.
  • Demonstrates and actively promotes a commitment to the mission of Trinity Health through behaviors aligned with its core values.
  • Maintains knowledge of applicable Federal, State, and Local laws and regulations, as well as Trinity Health's Organizational Integrity Program, ensuring adherence to ethical and professional standards.
  • Exhibits customer service excellence in all interactions.
  • Ensures accurate charge entry for patients in accordance with departmental practices, making necessary corrections as per charge capture policies.
  • Actively engages in ongoing education and communication regarding revenue management.
  • Proficiently utilizes electronic health records for documentation and care management throughout the shift.
