Position Overview:

Contingent Pharmacist

Afternoons/Rotating Weekends (every other weekend or every 3rd weekend)

Under the supervision of the Manager of Community Pharmacy Services and the Manager of Inpatient Pharmacy Services, along with the Clinical Pharmacy Manager, the incumbent will deliver clinical support and distribution services (pharmaceutical care) in collaboration with other healthcare professionals to ensure optimal, safe, and cost-effective medication therapy for patients.

Qualifications:

Minimum Education, Licensure / Certification and Experience Required:

A. Education
  • A foundational degree of B.S. in pharmacy from an accredited College of Pharmacy or equivalent is the minimum acceptable level of education.
  • A Doctor of Pharmacy degree and/or additional ACCP accredited certification is highly preferred.
B. Licensure / Certification
  • Registered pharmacist with eligibility for a Michigan Pharmacist and Controlled Substance license.
  • Current Basic Life Support (BLS) certification.
  • If the pharmacist is a member of RRT or responds to Codes, Advanced Cardiac Life Support (ACLS) certification is required.
C. Special Skills / Aptitudes
  • Interpersonal skills essential for effective communication with a diverse range of hospital healthcare personnel and patients.
  • Commitment to uphold activities that support the vision and mission of Trinity Health and the Department of Pharmacy, along with principles of continuous quality improvement within the organization.
  • Exceptional customer service orientation skills necessary for effective interaction with various levels of hospital personnel, external customers, and community groups.
D. Experience
  • Prior experience in hospital, retail, or long-term care settings depending on departmental function area.
Duties and Responsibilities:
  • Interprets and assesses all physicians' medication orders to ensure suitable drug therapy for each patient in accordance with legal and regulatory policies, rules, and standards.
  • Reviews, maintains, and screens patient medication orders and profiles for allergies, drug-drug interactions, drug-food interactions, and therapeutic duplications as well as out-of-range doses.
  • Engages with physicians, nurses, and other healthcare providers and patients to clarify orders that are unclear, incomplete, inappropriate, or could lead to medication errors, and recommends alternative therapies. Documentation of intervention activities is required.
  • Provides drug and poison information education programs to healthcare professionals in response to inquiries or in-service education.
  • Accurately prepares and dispenses medications and IV admixtures for both inpatients and outpatients.
  • Compounds and dispenses medications and injectables, recognizing that errors in compounding or dispensing may lead to significant patient morbidity and mortality.
  • Competency must be demonstrated prior to compounding or verifying compounded products.
  • Adheres to USP 797/800 Guidelines.
  • Directly supervises pharmacy technicians and interns in distributive functions and department operations and assists in annual performance reviews.
  • Assists in the training and orientation of new pharmacists, pharmacy technicians, and pharmacy interns.
  • Participates in training undergraduate and graduate students in operational or clinical areas of responsibility.
  • Maintains control and accountability for the handling of Controlled Substances, dangerous or toxic drugs, and investigational drugs as mandated by Federal and State regulations. Properly maintains all legally required records.
  • Engages in all pharmaceutical care activities as required, including patient rounds, therapeutic drug monitoring, patient education or counseling, admission drug histories, pain management, etc., as appropriate.
  • Makes timely and appropriate decisions using sound judgment to minimize drug costs and ensure the efficacy and safety of medication therapy.
  • Demonstrates and actively promotes an understanding and commitment to the mission of Trinity Health through actions consistent with the organization's values.
  • Maintains a comprehensive understanding of applicable Federal, State, and Local laws and regulations, as well as the Trinity Health Organizational Integrity Program, including the Standards of Conduct and Code of Ethics, to ensure adherence in a manner that reflects honest, ethical, and professional behavior.
  • Conducts oneself in a manner consistent with customer service expectations.
  • Ensures that appropriate charges have been entered for the correct patient, encounter, and date/time of service, with any required modifiers, in accordance with departmental practices. Makes corrections as needed per charge capture policy/practice.
  • Actively participates in ongoing education and communication regarding revenue management.
  • Assists with tracking and monitoring of equipment assigned to the unit and requests services for maintenance as needed.
  • Proficiently utilizes electronic health records for documentation, assessment, and care management, performing these activities concurrently with the provision of care throughout the shift.
