Description:
As a Contingent Pharmacist, you will work during afternoons and rotating weekends (every other weekend or every third weekend). Under the guidance of the Manager of Community Pharmacy Services, the Manager of Inpatient Pharmacy Services, and the Clinical Pharmacy Manager, you will deliver clinical support and distribution services (pharmaceutical care) in collaboration with other healthcare professionals to ensure safe, effective, and economical drug therapy for patients.

II Position Qualifications:
A. Education: A minimum of a B.S. in pharmacy from an accredited College of Pharmacy or equivalent is required. A Doctor of Pharmacy degree and/or additional ACCP accredited certification is highly preferred.
B. Licensure / Certification: Must be a registered pharmacist with eligibility for a Michigan Pharmacist and Controlled Substance license. Current Basic Life Support (BLS) certification is necessary. If the pharmacist is a member of RRT or responds to Codes, Advanced Cardiac Life Support (ACLS) certification is required.
C. Special Skills / Aptitudes: Strong interpersonal skills are essential for effective communication with a diverse range of hospital healthcare personnel and patients. A commitment to uphold the vision and mission of Trinity Health and the Department of Pharmacy, along with a dedication to continuous quality improvement, is crucial. Excellent customer service skills are necessary to interact effectively with various levels of hospital staff, external customers, and community groups.
D. Experience: Previous experience in hospital, retail, or long-term care settings is preferred, depending on the departmental function area.

III Duties / Responsibilities:

1. Interpret and assess all physicians' medication orders to ensure appropriate drug therapy for each patient in compliance with legal and regulatory standards.

2. Review, maintain, and evaluate patient medication orders and profiles for allergies, drug-drug interactions, drug-food interactions, therapeutic duplications, and out-of-range doses.

3. Communicate with physicians, nurses, and other healthcare providers to clarify unclear, incomplete, or inappropriate orders and recommend alternative therapies. Document intervention activities appropriately.

4. Provide drug and poison information education programs to healthcare professionals in response to inquiries or in-service education.

5. Accurately prepare and dispense medications and IV admixtures for both inpatients and outpatients, recognizing the significance of compounding and dispensing errors on patient safety.

6. Demonstrate competency before compounding or verifying compounded products and adhere to USP 797/800 Guidelines.

7. Supervise pharmacy technicians and interns in distributive functions and departmental operations, assisting in annual performance reviews.

8. Assist in the training and orientation of new pharmacists, pharmacy technicians, and pharmacy interns, as well as in training undergraduate and graduate students in operational or clinical responsibilities.

9. Maintain control and accountability for the management of Controlled Substances, dangerous or toxic drugs, and investigational drugs in accordance with Federal and State regulations.

10. Participate in all pharmaceutical care activities, including patient rounds, therapeutic drug monitoring, patient education, admission drug histories, and pain management, as appropriate.

11. Make informed decisions quickly to reduce drug costs while ensuring the efficacy and safety of drug therapy for patients.

12. Maintain a comprehensive understanding of applicable Federal, State, and Local laws and regulations, the Trinity Health Organizational Integrity Program, including the Standards of Conduct and Code of Ethics, to ensure adherence to ethical and professional standards.

13. Conduct oneself in a manner consistent with customer service expectations.

14. Ensure appropriate charges are entered for the correct patient, encounter, and date/time of service, making corrections as needed per charge capture policy.

15. Actively engage in ongoing education and communication regarding revenue management.

16. Proficiently utilize electronic health records for documentation, assessment, and care management while providing care throughout the shift.
