  • Receive and process referrals from agency personnel and community partners
  • Engage with potential participants and referrals to discuss program enrollment
  • Provide detailed information about the PACE program through various communication methods
  • Ensure that prospective participants are well-informed about the program and their rights
  • Assess eligibility for program participation
  • Contribute to enrollment meetings and share non-clinical insights
  • Maintain precise records related to intake and enrollment processes
  • Offer supplementary information for thorough evaluations
  • Coordinate initial assessments for potential participants
  • Keep open lines of communication with Program Management
  • Comply with regulations while delivering exceptional customer service
  • Suggest and implement enhancements for program efficiency
  • Carry out additional assigned responsibilities as needed

Required Qualifications:

A Bachelor's degree or higher in Social Work is required, with a Master's degree preferred.

3-5 years of experience in Social Work, ideally with elderly populations.

Excellent communication skills, attention to detail, and strong organizational abilities.

Capability to juggle multiple tasks and manage workload effectively.
