We are currently seeking a full-time Intake Coordinator for our operations. This role is essential in managing the comprehensive coordination of care for patients receiving home infusion therapy. The position is based in our Woburn, MA branch, with training conducted entirely on-site. Opportunities for partial remote work may be available after successful training.
Key Responsibilities:
- Compile and maintain the initial Patient Profile for each therapy, ensuring accurate distribution to the healthcare team.
- Manage incoming calls for the Intake Workgroup, documenting all communications appropriately.
- Facilitate effective communication with pharmacy, reimbursement, and delivery teams regarding patient referrals and ongoing issues.
- Collaborate with the Liaison/Care Coordination team to ensure smooth patient discharge processes.
- Document all patient-related information in clinical records, maintaining thorough and accurate updates.
- Communicate changes in patient supply needs to relevant teams to ensure continuity of care.
- Adhere to company policies and procedures while performing daily responsibilities.
- Develop a strong understanding of insurance coverage with guidance from senior staff.
Professional Qualifications:
- Excellent verbal and written communication skills.
- Strong organizational and customer service abilities.
- Attention to detail and a collaborative team spirit.
- Proficiency in Microsoft Office applications.
Preferred Experience:
- Familiarity with HIPAA regulations.
- 4-5 years of experience in healthcare administration.
- Associate's degree or equivalent education.
- Knowledge of medical terminology.
