Position Summary:

The Referral Management Specialist is responsible for overseeing and updating the status of referral orders initiated by healthcare providers. This role involves utilizing electronic tracking systems within the clinic's Electronic Medical Record (EMR) and Health Information Exchange (HIE) tools. The specialist ensures accurate documentation and prepares essential medical forms, including progress notes and lab reports, while managing the authorization process for referrals. The position emphasizes quality continuity of care and patient safety.

Key Responsibilities:

  • Adhere to organizational policies, HIPAA regulations, and applicable laws.
  • Collaborate with healthcare providers, medical staff, and external agencies to facilitate referrals.
  • Engage with patients to promote understanding and education regarding their referrals.
  • Maintain and track the status of referral orders using the EPIC EMR referrals module and other HIE tools.
  • Process referral orders linked to various health programs and services.
  • Utilize automated workflows to track authorizations and communicate updates through approved channels.
  • Ensure clear communication regarding referral statuses and maintain compliance with internal and external audit requirements.
  • Identify and address deficiencies in referral orders and coordinate with providers for necessary follow-ups.
  • Act as a liaison for patients and specialty providers, coordinating care transitions effectively.
  • Provide necessary information for cancer screening programs and ensure timely communication with relevant staff.
  • Report connectivity issues and assist in maintaining the EPIC Referral Module.
  • Foster positive relationships with patients, agencies, and clinic personnel to ensure quality service delivery.
  • Maintain logs for abnormal mammograms and coordinate appointment services for cancer detection programs.
  • Ensure compliance with data submission systems for reporting purposes.
  • Generate and route necessary documentation to appropriate departments.

Qualifications:

  • High School diploma required; college degree preferred or equivalent experience in healthcare.
  • Familiarity with Electronic Medical Records Systems.
  • Proficient in office equipment and electronic communication platforms.
  • Bi-lingual in English and Spanish.
  • Strong customer service skills, including communication and empathy.
  • Able to work effectively in a team-oriented environment and under deadlines.
  • Capable of managing multiple tasks in a fast-paced setting.
  • Willingness to travel as necessary, with a valid driver's license and insurance.
